Please do the following BEFORE you fill out the wholesale account form.

Fax (832-415-9313) or email in your State Based Sales and Use Tax Permit .
- For Alaska, New Hampshire, Montana, Oregon and Delaware AND outside the USA, send in proof of business e.g. your Business Licence.
- For ONLINE stores, make sure that your have an active and working web site - it is required to approve your account.
We ONLY ACCEPT applications that are COMPLETE. If you do not have all the required information, please wait to fill out the application when you do have it. Incomplete applications will not be reviewed or accepted.
Once we have received your COMPLETED application, along with your Sales Tax Permit or Proof of Business, we will approve your account and will be in touch with you within 3-5 Business Days.
You will have then have access to our wholesale pricing and will be able to place orders with us, once you have been approved and added into our wholesale customer system.
